ZIP codes are used to help you find convenient care. Entering your ZIP code allows us to show you the locations and services that are nearby. ZIP code information is not shared, stored, or used for any other purpose.

Not Seeing a Specific Medical Practice?

The medical practice you’re searching for may not appear because it:

  • Has fewer than four practitioners.
  • Has too few patients with available records.
  • Is included under a larger medical group.
  • Is a specialty practice not listed on the practice’s website.

We are the state’s most complete source of health and dental insurance data, representing nearly 70 percent of the population.

The database contains claims data from more than 30 commercial health care payers, Apple Health (Medicaid) and its five managed care plans, as well as Medicare fee-for-service (FFS) and Medicare Advantage -- the health maintenance organization (HMO) for Medicare members.
